Chronic kidney disease, a general term of most kidney diseases in the world can be divided into 5 stages in clinic on the basis of GFR, which is short for glomerular filtration rate. This kind of classification is accepted by many people all around the world. In China, there is also a kind of classification on the basis of serum creatinine for chronic kidney disease which can be divided into 4 kinds: compensation stage, discompensation stage, early stage of renal failure and end stage of real failure.
From the name, we can know that in the compensation stage, serum creatinine is normal. In this stage, patients generally have no symptoms, or they can only feel weakness, soreness of the waist, and increased urine at night.
In the discompensation stage, serum creatinine is 133 to 442 umol/L. In this stage, patients feel poor appetite, slight anemia and slight acidosis.
In the early stage of renal failure, serum creatinine is 442 to 707 umol/L. In this stage, the above mentioned symptoms become more obvious.
In the advanced stage of renal failure, serum creatinine is more than 707 umol/L. In this stage, patients begin to have heart failure, high K, bleeding in digestive tract, barriers in nervous centralis, and so on, which even threatens patients’ life.
Here some people will be confused why the serum creatinine is not high in the compensation stage. It is because the compensation ability of renal glomerulus. It means that in deed, there are some damaged renal glomeruli and in this case, the serum creatinine should be high. But our other normal kidney glomeruli can do more work to replace their work. They can do more to maintain normal range of serum creatinine.
In China, kidney disease can also be called a silent disease for many patients go to hospital when they are in uremia stage for they can not feel any symptoms or discomfort in daily life. In fact, in compensation stage, there is damage and there indeed exist differences in urine such as protein in urine, microscopic hematuria, etc. So timely diagnosis is not only necessary but also critical.
People are suggested to take examinations on blood and urine at least one time every year, which can help find out the minimal symptoms in chronic kidney disease.
